שאלון איתור לדיווח הורה/ מורה, המכיל 10 פריטים המתייחסים למיומנויות כתיבה שונות הנדרשות מהילד.
השאלון נועד להעריך את מיומנויות הכתיבה של הילד לצורך איתור קשיים בכתיבה והפניה לגורם הטיפולי המתאים. בנוסף עשוי לסייע למיקוד מטרות טיפול בתחום הכתיבה.
